Dates In Wrong Place

Note: the response below should usually fit  generally into the template laid out in the General Response format. If this issue was the only issue in the spreadsheet, you don't necessarily need to use the numbered list format ... since there's just one item in the list.

If dates are in the wrong place in the document -- for instance in the header somewhere, rather than over each column of data, then you can offer something like this:

It looks like you had your dates listed in [LOCATION IN DOCUMENT], but what we're looking for is to have a simple month and year listed at the top of every single column of accounting figures in your document. I usually use a simple format like e.g. "Jan 2012", so I went ahead and added this pattern into your document, creating headers wherever was needed. I also removed the original headers from the [ORIGINAL LOCATION] so that it wouldn't create any confusion with our software.

If they have "Total" on top of the column of data:

CFO Scoreboard requires that a date be listed at the top of every single column of accounting figures in your document. I simply replaced the header TOTAL in your documents with one of the date formats recommended.
